Who do you host with?

I'm looking for a webhosting service that will give me the following...

1) Great customer service
2) SSH (Shell access)
3) A plan for $10- 15 a month (paid monthly)
4) Allow adult content... Occasionally one of my members might post something in the forums that could be deemed inappropriate.

Any suggestions??
